The opener starts out with a male corpse dragged across the floor. The hologram that was moving the body and cleaning up the blood afterwards is having problems maintaining his image. Everything seems very dark and sinister.
After being roasted by Tom and Harry and then Janeway in the mess hall, Tuvok is unphased by their stories. In giving Tuvok a promotion, Janeway fastens the insignia on his collar that indicates that once again his rank is lieutentant commander.
In Tuvok's speech, he tells everyone that during his three years on Voyager he has come to respect a great many of them, but others he has learned to tolerate (looking in Harry's and Tom's direction). This amuses all the senior officers present. He ends his speech with the Vulcan sign for live long and prosper and tells them that as a Vulcan he shares their sentiment: "Live long and prosper"
B'Elanna immediately gets up, pats Tuvok on his back in a congratulatory way and takes off--fast. Tom sets his glass down, also pats Tuvok on the back and takes long strides to catch up to B'Elanna after calling her name.
Catching up to B'Elanna just outside the mess hall, Tom looks ill at ease as he laughs and looks at his feet before stating that it has been three days and they have not said a word to each other. After backtracking through the circumstances of her confession, he states that she probably
did not mean her confession of love.
She refutes this, thus showing him her internal fortitude to face up to this problem and get it behind her. She tells him that she does not expect him to reciprocate. Then she shows how nervous this is making her by her hand motions and her babbling in telling him to forget it.
This is when he steps in and tells her to "shut up." Holding one of her hands and placing his other hand on her shoulder, he precedes to kiss her. Moving his hand from her hand to her face and intensifying the kiss, they both appear to be enjoying it very much when the Doctor so rudely interrupts them in looking for Tom.
B'Elanna takes off with her words sounding very breathless in her exit (stage left).
Tom watches B'Elanna escape, as his eyebrows go up in quick contemplation, and catches his breath. The Doctor informs Tom that he has selected him as a replacement for Kes in sick bay during which Tom turns around several times to see if B'Elanna is still in sight. Tom is not thrilled at being the new nurse!
In the mess hall, Chakotay informs Harry that Seven and Harry are going to be working on the new isometric lab.
Janeway and Neelix are sitting down at the table discussing Neelix's arrangements to have a meeting with some aliens for tomorrow. Janeway wants him to consider this his first official assignment as ambassador.
Janeway and the Doctor are both summoned to the bridge by Ensign Culhain because of an automated distress call five light years away. When they reach the bridge, the HD-2-5 isomorphic projection (hologram) tells them there has been an accident. His crew is dead, he is alone, and he is asking for help.
The Doctor starts excitedly giving orders and tells Janeway that he will lead the away mission when Janeway finally puts an halt to his commands (Janeway's quote of the day) with "I don' t recall giving you a promotion today."
The Doctor only let's this dampen his feelings for a minute before arguing how he is the most appropriate one to go. He continues to plead his case and tells Janeway how he would take Lieutenant Torres with him because she knows the most about his moblile emmitter.
In cargo bay two, Harry informs Seven that they will start in jeffries tube 32-B. Their dialogue was meant to be humorous.
In the shuttle, B'Elanna informs the Doctor that Tom is a very responsible guy in response to the Doctor's pacing in the back of the shuttle and spoken concerns about leaving Lieutenant Paris in charge of sick bay.
The Doctor responds in telling her that she and Tom seem to have become "good friends." To which B'Elanna heatedly delivers a lecture to him about keeping his nose out of her business.
This leads to the Doctor's quote of the day: "Sorry, I didn't realize that I had struck a nerve. Perhaps you would like a tranquilizer."
B'Elanna gives him a narrow-eyed look and appeared to be ready to make her own cutting remark but was interrupted by a beeper on the console that indicated that they had found the ship that sent the distress call. In the Doctor's exuberance to meet this other hologram, he again starts giving out orders. B'Elanna exasperation with the Doctor is barely contained before they leave the shuttle.
When they beam aboard the ship, it is dark and the power seems to be shorting out because the lights are flickering and making crackling noises. The isomorph appears just behind the Doctor's shoulder and then disappears when B'Elanna brings the power back on line at a console.
While B'Elanna checks the Doctor's mobile emmitter, the isomorph picks up a tool that is similiar to our hammer and heads toward their backs. When he realizes from B'Elanna's and the Doctor's conversation that the Doctor is an isomorph like himself, he disappears again, thus dropping his tool with a loud bang. B'Elanna turns and picks up the tool from behind her when the isomorph reappears in front of them on the other side of the console that they are standing at. He quickly informs them that he did not mean to frighten them. He finds out B'Elanna is not a isomorph and looks at her with distrust.

Convincing him that she wants to help him, B"Elanna goes to another console to access his system and fix his malfunctioning.
He tells the Doctor that he left his home planet of Sorrus (spelling?) eight month ago with a crew of six. He also tells him that he is a HD-2-5 maintance unit with extreme hazard clearance. The isomorph is responsible for cleaning the reactor core and ejecting antimatter waste. He becomes very emotional about his crew becoming "sick" and dying.
B'Elanna interrupts him to get instructions on where to access his primary isometrics. She goes to an interface junction in another compartment on the some deck after being informed by him that she would be unable to access his primary isometrics on the lower deck due to it being flooded with antimatter radiation.
The isomorph is very inquisitive about the Doctor. Some of his statements lead the Doctor to think that he is socially inept.
Back aboard Voyager, Harry and Seven are busy when he checks her work and finds she has misaligned the optical assembly. Seven's quote of the day: "It must of been my humanity reasserting itself."
They go to access the main power supply. Harry saves her from her own stupidity and embarrassed himself when he grabs her shoulders to stop her from reaching into the power supply with her Borg hand.
On the alien ship once again, the Doctor and the isomorph is comparing the Doctor's mobile emmitter and the freedom it gives the Doctor to the isomorph's life of confinement. The Doctor starts scanning with an instrument out of his case when the isomorph spots some blood and starts scrubbing. When questioned by the Doctor, the isomorph tells him he is just sterlizing the ship.
B'Elanna jumps when the isomorph suddenly appears behinds her with no warning holding a plate of food. As he approaches her further, she stops him from stepping on an isomagnetic conduit and destablizing his matrix. The isomorph apoligizes for barging in and being so stupid. When B'Elanna begins eating the food, he compares her nibbling to that of a fish and is very complimentary and pleasant. He notices the conduits and things taken apart and becomes agitated when B'Elanna tells him she understands how he feels about the ship being a part of his body and soul. In a state of dementia, his remarks are very cutting about her being an organic and how disgusting he finds all organics. When he starts shaking his hands in her face, the tension is palpable.
Screaming at B'Elanna that she thinks she is the height of intellect in the universe, but is no better than any filthy animal, he appears to be going "over the edge" with the remark that he is ashamed to be made in man's image. In an abrupt change in demeanor, the isomorph apologizes for his conduct. B'Elanna excuses herself and gingerly eases around him and leaves.

The ensuing discussion between B'Elanna and the Doctor about the isomorph's behavior leads to B'Elanna quotes of the day. "Inept! He's a lunatic." And in response to the Doctor's reminder about his own behavior when first activated by the Voyager crew: "How could I forget? You were a major pain in...."
B'Elanna informs the Doctor that she is going to search the lower deck to see if she can access the isomorph's primary isometrics (she scanned the lower deck and found that it was not flooded with antimatter radiation). She tells the Doctor to keep the isomorph busy just before he appears holding his holographic fish (Spectrum) in a bowl. He tells the Doctor that he has programmed all sorts of things after B'Elanna has left.
Back on Voyager, Seven "damages" her hand and Kim takes her to sick bay. After Seven is treated by Tom and leaves, Harry attacks Tom's bedside manner. Tom accuses Harry of being too protective and having a crush on Seven. To which Harry at first denies and then admits that he feels maybe just a little that way, Tom advises him to be careful. Harry thanks him for his advice and leaves. Tom makes a noise of disbelief and says to himself out loud: "I just hope you take it" and shakes his head -- as if to say he knows his advice will be ignored.
On the other ship the Doctor is familiarizing the isomorph with all the systems controls so he can be more self-sufficient. (While below on the lower deck, B'Elanna with flashlight on wrist and tricorder in hand is searching for the isomorph's controls.)The isomorph invites the Doctor to leave Voyager so that together they can explore the galaxy.
In the eerie halls below, B'Elanna reaches the compartment with primary controls that she had been looking for, after being startled at the closing of the door, she starts turning power on to different controls. As the lights start coming on, she turns around and gasp at the gruesome sight of a man's head and arm hanging upside down outside the compartment window. Her agitation increases as more dead corpses can be seen at other windows.
Upstairs the isomorph is becoming erratic and tells the Doctor that they are the higher forms of life. Soon after this B'Elanna accesses his matrix and an alarm goes off that alerts him to what she is doing. Telling the Doctor that he will be back, he disappears. The Doctor takes off running.
Below the isomorph angrily confronts B'Elanna, so she puts both fists together and takes a powerful swing at him with such force that it hurls her across the room because her fists just go through his holographic body. He takes her by the shoulder and jerks her around. He proceeds to insert his hand inside her chest cavity amist cries of pain and agony. She finally reaches his controls and turns him off after what seems like an endless time of struggle. The Doctor arrives in great distess to find B'Elanna collapsed on the floor.
The return to Voyager at this time is suspenseful. Harry tries to entice Seven to go to the holodeck for a romantic evening. Sevens response is to offer to have sex (no feelings involved). Harry is embarrassed and calls it quits.
Returning to where they left off, the Doctor is assuring B'Elanna that she is safe and that she had not taken the isomorph emmitters off line any too soon. He informs her that he had found six corpses and all of them had been murdered. She had almost become number 7. He also tells her that her fourth ventricle had been perforated.
After helping her back to the command chamber, the Doctor goes to get her engineering kit (and seeing the holographic fish that would not have been there if all the emmitters had been off line), hears a thud and turns to see B'Elanna crumpled on the floor and the isomorph standing near her with his tool in his hand.
Sharing the Doctor's thoughts with the isomorph about his being unstable meets with the isomorph's immediate denial and prompts him to take a swing at the Doctor. He continues to rant about the Doctor being a liar. The Doctor provokes him again with the comment that this could be tedious, and he hits the Doctor's mobile emmitter and damages it so that the Doctor goes off line and disappears. The isomorph picks up the emmitter and almost in disbelief says,"Freedom."
B'Elanna groans, touches her forehead (where she is wounded and bleeding) and weakly gets up.
When the isomorph sees her getting blood everywhere, he announces that he is going to deactivate her. After this he does not say another word and the suspense is terrific in this cat and mouse game. The isomorph gets his just deserts when B'Elanna extends the isomagnetic conduit into his holographic body and we get some outstanding special effects as he is destroyed. Appearing drained of energy, she fixes the Doctor's mobile emmitter while sitting down and leaning against a wall. After bringing the Doctor back on line, he graciously helps her up. They are both very glad to leave.
Once again we are back on Voyager, Harry is giving the schematics for the isometrics lab to Chakotay. Harry is unable to get out of working with Seven because he is too humiliated to talk about their misunderstanding with Chakotay. Chakotay informs him that they (Seven and Harry) make a great team and he wants to keep them working together. Harry leaves with an embarrassed laugh. Chakotay laughs after Harry leaves in what seems to be amusement.
In sick bay, the Doctor has finished repairing B'Elanna heart and tells her that her pericardium is clean as a whistle which is more than he can say about his sick bay. Tom is apologetic about the mess. The Doctor just shakes his head in disbelief when Tom gives an account of all the ailments that he had to treat that day.
B'Elanna teases Tom with a question about being too tired to meet later. Tom not to be out done retorts with the question as to whether she thinks her heart can take it.
The Doctor runs his medical tricorder and announces that he is detecting elevated hormonal levels. If they do not take it easy, he will have to declare a medical emergency they are told.

Tom makes an excuse about checking on Harry who has had a nervous breakdown. When B'Elanna looks concerned and starts to inquire, Tom holds a hand up and tells her it is a long story and starts off when the Doctor stops him.
Telling Tom that he is to help him sterilize every square inch of the sick bay (being melodramatic), he starts sounding like the isomorph did about organics, much to B'Elanna apprehension. Then in a facetious abrupt change, he states that he is just kidding. In fact, he is having a change of heart about his fastidiousness and a "little clutter never hurt anyone" (and drops his hyperspray). With a final, "Sick bay should have a more organic touch, don't you think? to help our patients feel more at home," the Doctor exits to his office where he puts his feet upon the desk.
Tom, looking bewildered at this tirade from the Doctor, walks back over to B'Elanna who looks as first bemused and then put out as she crosses her arms and purses her lips. When Tom asks what has gotten into the Doctor, B'Elanna replies that it is a long story.
