REGISTRATION OF DEATH

1. Clear all hospital bills at Accident and Emergency Department or at Payment Counter.


2. Collect the Burial Certificate (Permit Penguboran) and the Death Report Form (Borang Lapuran Kematian) from the Ward.


3. Fill the the Death Report Form (Borang Lapuran Kematian).


4. Registration of Death is done at the

Jabatan Pendaftaran Kematian
Tingkat 1, Wisma Persekutuan
Jalan Bayam, 15200
Kota Bharu.

5. The following documents are required for registration

Death Report Form (Borang Lapuran Kematian)
Burial Certificate (original and 1 photocopy)
Deceased’s Identity Card
Informant’s Identity Card
Registration should be completed by the next working day.
