The following series of questions were kindly answered by 
Mullah Asgher, according to the Risala of Ayatullah Sistani.

Question:

1. It is known that in salaat, there are 5 wajib rukn:
   a.Niyat
   b.Takbiratul Ehram
   c.Qayam muttasil ba rukoo
   d.Rukoo
   e.Sajdatain

It is said that if any of the wajib rukn are missed deliberately or even
due to a lapse, then the salaat is batil (invalid). 

Now, concerning doubts during the prayer, among the doubts that are to be
ignored include: 

a. After having finished the salaat, a doubt as to whether one has gone
thru all the ritual or not. 

b. Doubt after passing of the event i.e. while in sajdah, a doubt about
having missed a rukoo. 

My question comes as to how to reconcile a doubt about the performance of
a wajib rukn which occured after the passing of the rukn?  Which takes
precedence: that a wajib rukn if missed for whatever reason will nullify
the prayer, versus a doubt that occured about such rukn after its passing? 
This truly has caused problems for me. I would appreciate any insight into
this matter. 

Answer:

Q1. The issue of reconciling the ruling about doubt and the ruling about
missing a rukn does not arise. As you have said, the doubt about the Rukn,
after its passing, is to be ignored. eg Masail no 1180. On the other hand,
missing a Rukn accidently or purposely will invalidate prayers only if you
are certain you have done it, not if you doubt about it after its passing,
or are unaware of it. I hope this is helpful. 

QUESTION:

2. What happens if while making wudu or ghusl, you accidently swallow
water before it is the time to break fast? Will this nullify your fast? Is
it necessary to make this day up? This occured to me while I was making
ghusl, but I suppose it can also apply for wudu. 

Answer:

Q2. If you swallow water accidentally, or even drink water forgetfully,
the fast is not invalid. Masail no 1584. Also see Masail no 1662. 

QUESTION:

3. If it is necessary to make ghusl, must wadu also be made for the all
the circumstances that necessitate that wudu be made? I am specifically
talking about ghusl janabat...when this has to be done, must wadu also be
made for circumstances that, irrespective of the janabat, would cause wudu
to be made? Or does the ghusl cover for the wudu as well? 

Answer:

Q3. After doing Ghusl for Janabat, Wudhu should not be done for prayers. 
Wudhu is not required after all Wajib Ghusls, except the Ghusl in some
cases of Istihadha. After the Ghusl for touching the dead body, it is
Makruh to pray without Wudhu. In cases of Mustahab Ghusls, it is better to
do Wudhu.  Masail nos. 397, 651-653. 

QUESTION:

4. If a Qadha prayer is made up, i.e zuhr or asr...and it is accidently
recited loudly instead of soft, and this was only realized after the
conclusion of the prayer, does this negate the prayer? 

Answer:

Q4. There is no harm in accidentally reciting loudly instead of softly, or
vice-versa. Masail no 1004.
