I pulled this album off the Rack, and the title is "Izipho Zam (My Gifts)", recorded 14 Jan 1969, issued as Strata East SES-19733, 'Dolphy Series 2'.
The song "Izipho Zam" is 28 minutes long, most of which features a simple chord sequence, with occasional all-percussion interludes. After about 20 minutes of that, a huge cacophony, much noisier than "Ascension", that lasts a few minutes, then back to the same old same old, and done. On side one there is a feature for vocalist Leon Thomas, yodeling away on "Prince of Peace": very nice, but no Sanders or other soloists. There is also "Balance", which switches back and forth from a quiet section to one of the manic intensity of that one part of "Izipho Zam".
Overall, I'd say it was not so much a "difficult listen", as it is not a very good listen. There is a amazing cast of musicians, however:

Unfortunately, there are really no solos from anyone; the music doesn't take advantage of these talents.
