We recommend that you first look at our Records Inventory to determine whether our facility houses the series of records and the dates which you believe are applicable. Once you have determined that the series and dates are available, it will simplify matters if you print out a records request form and then send payment of $5.00 (minimum charge) to:

If we determine that more research than half an hour will be required to fulfill your request, we will promptly notify you. The application form to research records is available in two different formats, HTM and PDF. While the PDF is more legible, you will need the Adobe Acrobat Reader to view and print forms which are stored in PDF format. The reader is free and can be downloaded by clicking here.
