DESE Curly-Coated Retriever

Puppy Questionnaire

This is not an interactive document but you can cut and paste it into Microsoft Word and send it as an email attachment or cut and paste it into an email and send it to us that way.  Thanks for taking the time to fill this out.

This questionnaire is designed to help us get to know you better so we can do a good job of matching puppies and owners. There are no “right” or “wrong” answers. The goal is to get a fix on what type of dog would fit best with your personality and lifestyle.  The more you elaborate, the better the match can be.  Every puppy is a living, breathing, feeling creature. While he or she is shaped in part by genetic influences, a puppy is shaped to an even larger degree by the environment he grows up in. We give puppies the very best head start that selective genetics, careful nurturing, and proper socialization can give—the rest is up to you. You will control the quality of life for this dog for the rest of his/her life; that’s a big responsibility. It can also be one of the most rewarding experiences you will ever have. We will always be available for consultation or to help steer you in the right direction to get any help you need.
