Zuid (South) Holland Ancestor Document Photocopy Order Form
The service I am offering you is making photocopies of your Zuid Holland ancestor's documents. I will take the information you provide and find their original document on microfilm. When I am positive it is your ancestor I will make a copy and mail it to you.

If you are interested in this service please fill in all the questions below… do NOT leave any question empty. It is very difficult to find a person's document when I do not have all the facts. If you do not know the requested information and can't fill in ALL the blanks then please do NOT request for me to find and make a copy of your ancestors document.

I do not do research for others... this is strictly a document copying service. My fee for this service is $3.00 (USA) per document copied.

Note: At this time I will only make copies from the Zuid (South) Holland archives.

The years I will make copies for are from 1695 to 1900. If the year you are requesting is missing in the archives then your money will be refunded.

If you have filled out the entire questionaire and I cannot find your ancestor's document by using the information you have provided, I will refund your money. However, if a form is sent to me without the necessary facts and I cannot find your ancestor's document because of these missing facts then there will NOT be any refund.(Your ancestor's correct names, their birth, marriage or death dates, depending upon which photocopies you request are the "facts" I am referring to above.)
 Please fill out a new form for each document photocopy you request. Make a copy of this form and mail it (via regular Postal service) along with your USA $3.00 check or USA/International Money Order made out to: Joyce Jensen. Please include with your order a self addressed, stamped envelope.
