National Archives Form 14027
This is what the form looks like that a person uses to request Military Records from either the Revolutionary War, War of 1812, Civil War and other early military conflicts.  Once the necessary information is completed (read the instructions on the form), submit the form to Staff, time stamp it and place it in the basket to be pulled.  As you can see, you may request  three (3) separate types of records with this form; Military, Pension and Bounty Land Records.  BUT!  Only one request per form. 
If what you are looking for is a Pension Record, remember, after you have searched and jotted down all of the pertinent information and numbers from the microfilm, make sure all information is completed.  This includes the person's Application, Certificate and Other Numbers (section 18 a through e).
