In the search for the Ark, Indiana Jones (Harrison Ford) drops into the snake-infested Well of Souls, only to come face-to-face with a king cobra. Yes, the snake was real, and to protect Ford, a pane of glass separated him from the angry reptile and his buddies. Not only can you see the reflection of the cobra's white belly in the glass, but Kevin J. Turner reports that if you look close, you'll actually see the snake smack into the glass when it lunges at Indy.
Ransom (1996):
Not even Mel Gibson can distract a dedicated slip-spotter from her duty. In two separate scenes in Tom Mullen's (Gibson) penthouse, the boom mike dips into view.
And here's a slip that only a true New Yorker (or someone who was recently there, like my parents, who pointed this one out) would spot. The Mullens' penthouse is on FDR Drive, on the East Side, near the river. This is establised at the beginning, and confirmed several times by shots out the windows or over the balcony. However, when Tom leaves his apartment and crosses the street to talk to reporters, he's suddenly in front of the Guggenheim Museum, roughly a mile away in Central Park.
Reservoir Dogs (1992):
In the scene where Mr. Pink (Steve Buscemi), Mr. White (Harvey Keitel) & Nice Guy Eddie (Chris Penn) leave the warehouse to get the stashed diamonds, Mr. Blonde (Michael Madsen) is sitting on top of a huge box. Blonde sticks his cigarette in his mouth, jumps off the box, takes off his coat and tosses it down. The whole time he's talking to the hostage cop, the cigarette is in his mouth. Immediate camera angle change - and the cigarette's now in his hand. Bridgett spotted the slippery smoke.
Robin Hood (1973):
At the archery tournament, Prince John is so intent on finding Robin Hood, he doesn't notice his jumpy rings. First, he isn't wearing any. Then, he has three rings on his left hand and one on his right, all with large jewels. The jewel disappears from his right hand, then reappears. His right hand suddenly has three rings, then one, then three, then one. Finally, all the rings disappear completely. And, later, when he's asleep in bed, the rings appear briefly, then disappear again.
You'd think a leg cast would be a bit more stable than rings. But Otto has a cast on his right leg in the blacksmith shop, then it's on his left as Friar Tuck helps him escape from jail, then it switches a couple more times before settling back on his right leg at the wedding.
Robin Hood: Prince of Thieves (1991):
When Azeem (Morgan Freeman) helps Robin (Kevin Costner) stand up on the beach he offers him his left hand. When the camera angle changes his right hand is extended.
The Rock (1996):
During the car chase the Ferrari gets its windshield smashed at least twice, but later the windshield is fine.
Also during the car chase, the Hummer hits a telephone pole and it stops. In the next shot, it's driving away.
John Patrick Mason (Sean Connery) was captured in 1962 with the microfilm and arrested. At the end, Stanley Goodspeed (Nicolas Cage) asks his wife, "Do you want to know who killed JFK?".
JFK wasn't killed until 1963!
The Rocky Horror Picture Show (1975):
The famed cult film holds the record for longest run in an American movie theater. After that many viewings, you know a whole lotta flubs are going to pop up. So many, in fact, that I had to put them in a bulleted list! Special thanks to my close friend Mandy McCarthy for bringing these to my attention, and to P7A77's official RHPS FAQ page for confirming them.
After the wedding photo, Ralph's mother (a brunette woman) and father (a bald man with a white tuxedo) run up the church steps twice.
The chalk heart that Brad Majors (Barry Bostwick) draws on the church changes shape in different shots.
Janet Weiss' (Susan Sarandon) purse moves places when she puts it on the church steps during "Dammit Janet".
The Criminologist (Charles Gray) states that the film's events happened on a late November evening. However, in the car, Brad and Janet are listening to Richard Nixon's resignation speech. Nixon resigned in August, in the daytime.
The windshield wipers on Brad's car are out of sync in different shots.
The Transylvanian with the Hors D'oeuvres during "Time Warp" puts it down in one shot, and in another shot
suddenly has it again.
Columbia's (Little Nell) hat is on the ground immediately before her tap-dance, but when she starts dancing, it's on her head.
The red carpet mysteriously disappears for Columbia's tap-dance.
When Janet faints at the beginning of "Sweet Transvestite", a different hand is raised to her head in different shots.
Also, she continues to scream even after she has supposedly fainted.
Shawn McHorse, a member of the Queerios cast in Austin, Texas, pointed out a mistake that would likely only be noticed by someone watching Riff-Raff (Richard O'Brien) very carefully: when Dr. Frank-N-Furter (Tim Curry) struts up the aisle to his throne during "Sweet Transvestite", Riff-Raff is standing behind both Magenta (Patricia Quinn)and Columbia. But when Frank walks back down the aisle a few seconds later, Riff-Raff is in front of them.
Brad's shirt is unbuttoned in one shot, and then buttoned again when Magenta is undressing him.
One day during filming, the technical crew had an Easter Egg Hunt on the set. Rumor has it that three eggs can be seen: one under Frank's throne, one as the group ascends in the elevator, and the third is said to be where the lightbulbs go on one of the gargoyles in the main room.
When building the laboratory set, the crew forgot to build a door. (That's why Dr. Scott comes crashing through the wall.) Therefore, it would seem that the only way to enter the lab is through the elevator. However, when Brad and Janet arrive in the lab, all of the Transylvanians are already there. Either they all squeezed into the elevator at once, or there was a secret enterance.
Riff-Raff turns a wheel clockwise to lower a palette of colored goo during the "creation scene". When Rocky (Peter Hinwood) grabs the palette, Riff-Raff is still turning the wheel clockwise, but now the palette is going up. In the next shot, the wheel is being turned counter-clockwise. Likely the first shot was existing footage, since it matched a previous shot of the palette being lowered, and nobody noticed that it turned the wrong way.
The position of Rocky's arms and the types of moves he's doing change in different shots during "Charles Atlas Song".
Eddie's (Meatloaf) bracers disappear at one point while he is rolling around on the floor with Columbia.
As Eddie chases the Transylvanians around the lab on his motorcycle, one of them knocks the head off a statue. Duncan Campbell noted that the head is back on a few seconds later. It's possible there's footage of it being replaced, but if there is, we haven't seen it.
Columbia of the Sapphire Slippers from the Translucent Dreams Cast in Edison, NJ, found a slip when Frank is attacking Eddie. There is a shot of Frank with Columbia in the background slowly putting her hands up to her face. In the next shot, a closeup of Columbia, her hands are down and she puts them up to her face again.
There is blood in the freezer before Frank attacks Eddie with the pick.
While he's activating the magnet, Frank starts to raise his left leg, but when the camera shot changes, his right leg is
lifted.
The first shot of Dr. Everett V. Scott (Jonathan Adams) going up the stairs shows a piece of carpet turned over by the wire pulling the
wheelchair. Many people also think that the second shot is a re-take of the first, but if you look, the paintings and
such on the walls are different.
During the toast at dinner, Riff-Raff's hand can be seen pulling the napkin out of Dr. Scott's glass, yet
he pulls it out again a few moments later. Right after Frank's toast, the napkin is back in the glass, only to be gone
again a bit later.
Riff serves Brad a slice of Eddie in a close-up of Brad, and then again in a wide shot - yet Brad only ends up with
one slice on his plate.
Rumor has it that during the dinner scene, Bostwick accidentally stepped on or hit Sarandon (watch her face when he says "Just what exactly are you implying", and subsequent shots), and that she later stepped on his foot with her heel after the floor show (watch when they're backing up after Riff and Magenta come through the door). Only the second one has been confirmed (by Bostwick himself on the footage from the SE laserdisc).
During "Planet Schmanet, Janet", Frank pulls down Janet's bra strap, but it's back in place in the next shot.
At the beginning of the floor show, when Columbia's doing her solo, as she says, "Rose tints my world," her bustier slips, exposing her chest. You can see her trying to subtly push it back up as she dances.
The tattoo on Frank's arm is washed off in the pool during the floor show, reappears for the last bit of the floor
show, is washed off again at the end of the floor show, and is on again at the end of "I'm Going Home".
Brad and Janet are helping Dr. Scott out of the castle without a wheelchair, but Dr. Scott is lying on a broken
wheelchair during "Superheroes".

Romeo + Juliet (1996):
When Balthasar (Jesse Bradford) tells Romeo (Leonardo DiCaprio) that Juliet (Claire Danes) is dead, Romeo freaks out and the wind almost blows his unbuttoned shirt off. He then falls to the ground and cries, but when he stands up his shirt is buttoned.
In the scene where Romeo and Juliet kill themselves, Romeo blinks after he is dead.
Romy and Michelle's High School Reunion (1997):
When Heather Mooney (Jeanene Garafalo) tells Romy White (Mira Sorvino) that she invented fast-burning cigarette paper, there's a close-up of her cigarette rapidly burning down almost to the filter. But, in the next shot, the cigarette has magically grown back.
