There isn't too much to be said about blindfold safety, because under normal
circumstances blindfolds simply aren't dangerous. A few simple points, though:
Excessively tight blindfolds are a bad idea. They should be snug, and tightening a bit for
some discomfort is OK, but tightening with brute force can damage the eyeballs and cause
abrasions on the bridge of the nose.
Obviously you must take extra care of someone who's blindfolded and still free to move
around. You don't want them falling over or bumping into things.
When removing the blindfold, dim the lights, if possible. Don't whip it off in a brightly
lit room without warning. At least you should give your partner a chance to close their
eyes.
Wearing a tight ear-covering blindfold for a longer period of time (e.g. overnight) makes your ears hurt slightly when it's removed. This is a bit uncomfortable, but not all that painful, and it passes quickly.
Note that as with any kind of bondage, even moderatly tight blindfolds leave marks that
can take up to an hour to disappear. There's nothing "unsafe" about this, but it
can be tricky to explain if you get an unexpected visit: it's not easy to hide!
[Additional note: I had a nasty experience the other day. After spending the night wearing
a woolen felt scarf as a blindfold, I got an ugly rah on the bridge of my nose. It took
some days before it disappeared completely, after developing some pimples along the way.

Finally and importantly: Note that you should never blindfold anyone without their express
agreement (actually this should go for any kind of sexual activity). Even with consent,
you should watch out for signs of psychological stress. Blindfolds can affect people very
powerfully. This is especially true of claustrophobes, but anyone, even people who are
used to blindfolds, may get into a state of fright when blindfolded. If you notice this
happening, remove the blindfold (after warning your partner), remove any bondage if
required, and comfort them as caringly as you can.
