港・九竜湾の民間団地アモイガーデン(淘大花園)で発生した新型肺炎の重症急性呼吸器症侯群(SARS)の集団感染で、香港政府は17日、SARSウイルスを含んだ水しぶきや水蒸気が排水管を通じて下水管から浴室に流れ込んだのが原因とみられる、と発表した。 衛生福祉食物局によると、排水管にはU字に曲がった部分があり、水がたまって害虫や臭気を遮断する仕組み。しかし、水がない状態で浴室を密閉し、換気扇を回すと、気圧の変化によって下水管内の空気が浴室に入ることを確認した。下水管から水しぶきや水蒸気に混じって各戸の浴室にウイルスが拡散したらしい。

If you think you (or someone in your family) might have SARS, you should:

Consult a health care provider as soon as possible.
Cover your mouth and nose with tissue when coughing or sneezing. If you have a surgical mask, wear it during close contact with other people. A mask can reduce the number of droplets coughed into the air.

If you have SARS and are being cared for at home, you should:

Follow the instructions given by your health care provider.
Limit your activities outside the home during this 10-day period. For example, do not go to work, school, or public areas.
Wash your hands often and well, especially after you have blown your nose. Cover your mouth and nose with tissue when you sneeze or cough.
If possible, wear a surgical mask when around other people in your home. If you can't wear a mask, the members of your household should wear one when they are around you.
Don't share silverware, towels, or bedding with anyone in your home until these items have been washed with soap and hot water.
Clean surfaces (counter or tabletops, door knobs, bathroom fixtures, etc.) that have been contaminated by body fluids (sweat, saliva, mucous, or even vomit or urine) from the SARS patient with a household disinfectant used according to the manufacturer's instructions. Wear disposable gloves during all cleaning activities. Throw these out when you are done. Do not reuse them.
Follow these instructions for 10 days after your fever and respiratory symptoms have gone away.

If you are caring for someone at home who has SARS, you should:

Be sure that the person with SARS has seen a health care provider and is following instructions for medication and care.
Be sure that all members of your household are washing their hands frequently with soap and hot water or using alcohol-based hand wash.
Wear disposable gloves if you have direct contact with body fluids of a SARS patient. However, the wearing of gloves is not a substitute for good hand hygiene. After contact with body fluids of a SARS patient, remove the gloves, throw them out, and wash your hands. Do not wash or reuse the gloves.
Encourage the person with SARS to cover their mouth and nose with a tissue when coughing or sneezing. If possible, the person with SARS should wear a surgical mask during close contact with other people in the home. If the person with SARS cannot wear a surgical mask, other members of the household should wear one when in the room with that person.

“The World Health Organization (WHO) today began recommending that persons travelling to Hong Kong Special Administrative Region of China and Guangdong Province, China consider postponing all but essential travel. This updated travel advice comes as a result of new developments in the multi-country outbreak of severe acute respiratory syndrome (SARS).”2003.04.02

Chinese authorities had previously reported 792 cases and 31 deaths in Guangdong Province in the reporting period of 16 November 2002 to 28 February 2003. The cumulative total of SARS cases in Guangdong from 16 November to 31 March now stands at 1153 cases and 40 deaths.

香港の保健省は本日(4月1日)、これ以上の重症急性呼吸器症候群(SARS)の感染拡大を防ぐために、空前の隔離命令を出した。隔離命令は、厦門ガーデンのEブロックの住人達に、4月9日まで自宅から出ないようにと言うものである。

SARS患者の数が、ここ数日間の間にこのビルで連続して急激な増加を示したことを受けて、隔離命令を出す決定がなされた。昨土曜日に香港で入院した新規のSARS患者45例中22症例が、この住宅団地の住人であったことが判明したため、厦門ガーデンにおける集団発生の懸念が浮上した。日曜日には、SARSの可能性で入院した新規患者の60例中36例が厦門ガーデンの住人であった。

香港の保健当局は本日、疾病の報告が始まって以来累積で213人の厦門ガーデンの住人が、SARSの疑いで入院したことを公表した。香港の集団発生は、3月12日に保健担当官がPrince of Wales病院で異型肺炎の症例の集積を認知した時に始まった。

この集団発生で213人の厦門ガーデンの住人が感染し、そのうち107人がEブロックの住人であった。これに加えて、これらEブロックの107人の住居のほとんどは、垂直関係の配置になっていた。

このような事から、SARSは香港において異なったパターンで拡大しているが、依然として感染者の分泌物(飛沫など)を介する密接なヒトーヒトの接触によると考えられている。WHOの疫学者たちは、この起因ウイルスを含む患者分泌物が何らかの方法で、各部屋又は家庭を結ぶ共用のシステムに侵入したのではないかと言う可能性を考えている。これまでに報告され、良く知られている直接患者に向かい合う形の接触に、この形の伝播が加わったと考えられる。

今月初めに香港の疫学者たちは、メトロポール・ホテルで集団発生が起こった2月15日から23日の期間の宿泊客と訪問者の間での一般的でない伝播経路を確認した。ひとつの階の宿泊客と訪問者が、トロントとシンガポールへのSARSの感染拡大の原因と、香港のPrince of Wales病院の集団発生を引き起こしたと考えられる。このとき、ホテルのスタッフは全く発症していない。厦門ガーデンは九龍地区の、10棟の35階建てビルからなる大きな住宅団地で、約15,000人が住んでいる。

WHOは感染者が出たEブロックの住居環境から試料が採取され、この明らかな垂直方向への感染の広がりについて、説明ができるかどうかを調査中であるとの報告を受けている。

調査そのものは複雑であるが、環境からの試料の検査結果は数日で明らかになる予定であり、Eブロックの感染源と病原体の伝播機構の特定に役立つと考えられる。 香港の疫学者たちにより、厦門ガーデンの「発端者」と考えられる症例が特定された。この患者は現在Prince of Wales病院に入院しているが、入院前に数回に渡り、Eブロックに住んでいる兄弟のひとりに会いに来ていた。

SARS virus close to conclusive identification, new tests for rapid diagnosis ready soon On 17 March, WHO established a global network of leading laboratories to collaborate in the identification of the causative agent of severe acute respiratory syndrome (SARS).
Early on, labs in the network narrowed the search to members of the paramyxovirus and coronavirus families. Findings shared by network labs earlier this week began pointing, with increasing consistency, to coronaviruses.
“Data from many network laboratories indicate that a coronavirus is the primary cause of the disease,” said WHO virologist and epidemiologist Klaus Stohr. “This virus is unlike any known human or animal member of this virus family. It is consistently found in specimens from SARS patients from many countries. It has been isolated in cell-culture. We are very close to knowing for sure.”

World Health Organization issues emergency travel advisory 15 March 2003 -- During the past week, WHO has received reports of more than 150 new suspected cases of Severe Acute Respiratory Syndrome (SARS), an atypical pneumonia for which cause has not yet been determined. Reports to date have been received from Canada, China, Hong Kong Special Administrative Region of China, Indonesia, Philippines, Singapore, Thailand, and Viet Nam. Early today, an ill passenger and companions who travelled from New York, United States, and who landed in Frankfurt, Germany were removed from their flight and taken to hospital isolation.

WHO issues global alert over cases of atypical pneumonia 12 March 2003 -- The alert follows reports from the Department of Health, Hong Kong Special Administrative Region (SAR), China, on an outbreak of pneumonia in one of its public hospitals. WHO is working with authorities to control the outbreak in Hong Kong SAR and a similar outbreak in Hanoi, Viet Nam.
