I've named this uncataloged cachet after who it was addressed to.  I have no idea if Mr. Smith actually created this nice three colored cachet.  The small image in the cachet features a stethoscope, prescription form and a hypodermic needle.  If you can provide any of the history behind this cachet, please drop me a line at smith@in.net.
