CASE MANAGEMENT COMES TO T.O.U.C.H.

T.O.U.C.H. has been awarded a grant from Ryan White Title I to provide grant funded case management to our consumers. T.O.U.C.H. has a long history of providing case management, although this was never funded in the past. Case managers are available Monday through Friday by calling the office and by beeper after hours and on weekends. In order to receive case management at T.O.U.C.H. you must be infected or affected by HIV/AIDS and be willing to work on a plan of care. Case managers are the link between the consumer and services in the community. The case manager can be an advocate for entitlements such as Public Assistance and Social Security, refer the consumer for services such as medical visits, testing, telephone, utilities, and job training just to name a few. Case managers also provide much needed support to our consumers.
