Accident Summary:

On short final the engine failed. The aircraft turned right and landed in a wet rice paddy and skidded into a dike. The aircraft tilted causing the tail rotor to enter the water. The 90 degree gearbox separated from the aircraft. The main rotor blades struck an embankment.

Accident Summary:

 THE AIRCRAFT CRASHED INTO A RICE PADDY AT XS 576789, RVN, AT 0845 ON 6 JUNE 1969. THE AIRCRAFT DEPARTED FROM BEN LUC AT APPROXIMATELY 0840 ON 6 JUNE 1969, ENROUTE TRAI CU ON A ROUTINE COMBAT SUPPORT MISSION. UPON LEAVING BEN LUC, THE AIRCRAFT COMMANDER'S ARTILLERY CLEARANCE DIRECTED HIM TO FLY UP THE WEST SIDE OF THE RIVER TO TRAI CU. JUST AS THE AIRCRAFT CROSSED THE RIVER, A VIOLENT JOLT SHOOK THE SHIP AND A LOUD BANG WAS HEARD BY THE CREW. THE AIRCRAFT SKIDDED TO THE LEFT AT ABOUT A 45 DEGREE ANGLE. THE AIRCRAFT COMMANDER ENTERED AUTOROTATION AND PROCEEDED TO GET HIS EMERGENCY CALL TRANSMITTED. A MONEY SATCHEL CARRIED BY LT SHAPPOCK FELL OUT THE LEFT DOOR OF THE HELICOPTER AT THIS TIME. THE AIRCRAFT AUTOROTATED WELL AND DECELERATION WAS ACCOMPLISHED AT ABOUT FORTY FEET. BUT WHEN CUSHIONING PITCH WAS PULLED, THE SHIP STARTED TO SPIN IN A CLOCKWISE DIRECTION. THE SPIN WAS STOPPED WHEN 90 DEGREES COMPLETE BY THE LEFT SYNCHRONIZED ELEVATOR STRIKING THE GROUND AND RIPPING THE TAIL BOOM ALMOST FREE FROM THE REST OF THE SHIP. THE MAIN ROTOR STRUCK THE GROUND ON THE LEFT OF THE SHIP AND PULLED THE TRANSMISSION DOWN THROUGH THE CARGO AREA PINNING LT. J. G. SHAPPOCK BENEATH IT. WHEN THE AIRCRAFT CAME TO REST IN A WET RICE PADDY, THE SKIDS WERE COMPLETELY OUT OF SIGHT IN THE SOFT MUD. THE TRANSMISSION HAD TORN DOWN THROUGH THE ROOF OF THE AIRCRAFT. THE TAIL BOOM LAY PERPENDICULAR TO THE REST OF THE SHIP, POINTING THE DIRECTION OF FLIGHT BEFORE THE IMPACT. THE TAIL BOOM WAS HELD TO THE SHIP ONLY BY THE SKIN ON THE RIGHT SIDE.\\
