Accident Summary:
THE AIRCRAFT WAS ENROUTE FROM CAM LY AAF TO AN LZ SOUTH OF THAT LOCATION. THE SUBJECT AIRCRAFT, SERIAL NUMBER 66-15115, WAS THE TRAIL SHIP IN A FLIGHT OF TWO UH1C TYPE AIRCRAFT AND TWO UH-1H TYPE AIRCRAFT. APPROXIMATELY FIVE MINUTES INTO THE FLIGHT, AND AT AN ALTITUDE OF APPROXIMATELY 600 FEET AGL, THE CREW HEARD A LOUD "POP" SOUND FROM THE ENGINE COMPARTMENT FOLLOWED BY A VIOLENT YAW TO THE LEFT. BELIEVING THE ENGINE HAD FAILED, WO1 ROWE, WHO WAS AT THE CONTROLS OF THE AIRCRAFT, INITIATED AUTOROTATION. UPON CHECKING HIS INSTRUMENTS HE WAS ABLE TO CONFIRM AN ENGINE FAILURE AS EVIDENCED BY THE N1 NEEDLE SO A PASSING THROUGH 50% AND DROPPING RAPIDLY. ALSO, A NUMBER OF CAUTION LIGHTS WERE ILLUMINATING AS WHEN SHUTTING DOWN. AT THIS TIME THE PIC INFORMED THE ACCOMPANYING AIRCRAFT AND THE CREW THAT THEY HAD SUFFERED AN ENGINE FAILURE AND WERE GOING DOWN, AND THEN DEVOTED HIS ATTENTION TO SELECTING A SUITABLE LANDING AREA. HE CHOSE AN OPEN FIELD ON TOP OF A HILL, THE LONG AXIS OF THE FIELD BEING APPROXIMATELY 060 DEGREES, THE SAME HEADING HE WAS USING INTO IT. THERE WAS A ROAD ON ONE SIDE WITH TREES ON THE OTHER SIDES, A SLIGHT INCLINE TO HIS RIGHT, AND PREVAILING WINDS OFF HIS RIGHT NOSE AT AN ANGLE OF 080 DEGREES. AS THE AIRCRAFT NEARED ITS INTENDED LANDING AREA THE PIC FELT HE WAS GOING TO OVERSHOOT AND CONTINUE INTO THE TREES AT THE FAR END, THUS HE FLARED THE AIRCRAFT TO SLOW HIS FORWARD MOTION AND THEN LEVELED OUT WHEN HE REALIZED THE INTENDED AREA WOULD BE REACHED. DURING THIS TIME THE CREW CHIEF AND GUNNER THREW OUT MOST OF THE M-60 DOOR GUN AMMUNITION, AND THE CO-PILOT JETTISONED THE ROCKET PODS. DROPPING RATHER RAPIDLY, THE PIC PULLED PITCH AND LANDED HARD, FIRST ON THE RIGHT SKID, THEN ON THE LEFT SKID. HE THEN APPLIED RIGHT FORWARD CYCLIC TO KEEP THE AIRCRAFT ON THE INCLINE. THIS RIGHT FORWARD CYCLIC MOVEMENT CAUSED THE AIRCRAFT TO NOSE OVER RESULTING IN THE MAIN ROTOR BLADES STRIKING THE GROUND. THE CRASH SEQUENCE TERMINATED WITH THE AIRCRAFT LYING ON ITS RIGHT SIDE APPROXIMATELY 90 DEGREES TO THE LEFT OF THE ORIGINAL INBOUND HEADING.\\
