During flight, a complete loss of tail rotor was experienced. The pilot attempted to return to the airfield, but the aircraft would not cease turning and would not streamline. An autorotation was made to an open area damaging the aft cross tube. The pitch change control rod sheared off

Received casualty reports from 92nd AC(AH): Aircraft UH-1H (66-16461) was on a troop insertion mission when upon entering the LZ ship began to vibrate, crashed but did not burn. SP4 David Todd Jr. RA 53846573, of 92nd received a lacerated right lower lip. Incident occurred at 1600 hours
