Accident Summary:
Pilot began pickup of aircraft and began a roll around longitudinal axis to the right. The pilot applied left cyclic to counteract roll. Aircraft continued to roll until blades contacted the ground. Pilot lowered the collective to get the aircraft on the ground but it continued to roll. Aircraft came to rest on right side with transmission and rotor head separated from aircraft

Accident Summary:
THE AIRCRAFT HAD DEPARTED FSB BASTOGNE AT APPROXIMATELY 1155 HOURS, WITH 400 POUNDS OF CARGO, ONE PASSENGER AND APPROXIMATELY 600 POUNDS OF FUEL ON BOARD. THE FLIGHT WAS 2 KM SOUTH WEST OF FSB BASTOGNE, TAKING APPROXIMATELY FIVE MINUTES FROM TAKE OFF TO LANDING. THIRTY-FORTY SECONDS SHORT OF THE LANDING ZONE, THE AIRCRAFT COMMANDER ASKED THE GROUND UNIT BY FM RADIO TO IGNITE A SMOKE GRENADE FOR IDENTIFICATION, WIND DIRECTION , AND VELOCITY. THE GROUND UNIT IGNITED A SMOKE GRENADE (VIOLET IN COLOR) AND THE AIRCRAFT COMMANDER AND PILOT ESTIMATED THE WIND AT 5-10 KNOTS. THE WIND WAS COMING FROM 260-270 DEGREES PRESENTING A DIRECT HEAD WIND IF LANDING WERE MADE TO THE WEST. THE CREW DECIDED TO LAND ON A HEADING OF 260 DEGREES. A HIGH RECONNAISSANCE WAS PERFORMED AT 500' AGL WHILE THE LANDING ZONE WAS CIRCLED TWICE. THE AIRCRAFT COMMANDER AND PILOT EVALUATED THE LANDING ZONE AS MARGINAL IN SIZE, COVERED WITH TREE STUMPS, AND LITTERED WITH FALLEN TIMBER. AT THIS POINT IN THE FLIGHT THE AIRCRAFT COMMANDER PHYSICALLY TOOK CONTROL OF THE AIRCRAFT JUST PRIOR TO TURNING FINAL APPROACH AND EXECUTED A STEEP APPROACH TO A HOVER IN THE LANDING ZONE. NOTE: THE WIND WAS DETERMINED TO BE 5-10 KNOTS FROM 260-275 DEGREES. THE LONG AXIS OF THE LANDING ZONE RAN 260/080 DEGREES. NOTE: THE CREW WAS STRONGLY CONSCIOUS OF POSSIBLE STUMP DAMAGE TO AIRCRAFT. CONSEQUENTLY THEY WERE APPREHENSIVE OF THE TOUCH DOWN AREA. THEIR UNIT HAD AN AIRCRAFT RECEIVE STUMP DAMAGE WITHIN THE PAST TWO DAYS. THE EM CREW BEGAN VERBALLY CLEARING THE AIRCRAFT ON INTERCOM FROM THE POINT IN THE APPROACH PATH THAT TREE TOP LEVEL WAS REACHED, UNTIL THE ACCIDENT. WHEN THE AIRCRAFT CAME TO A HOVER THE CREW CHIEF AND GUNNER BEGAN TO CLEAR BENEATH THE AIRCRAFT FOR FALLEN TIMBER AND STUMPS. TO DO THIS THEY WERE LEANING OVER LOOKING UNDER THE AIRCRAFT. DURING THIS PERIOD WO KENNY WAS MONITORING THE ENGINE/TRANSMISSION INSTRUMENTS. THE AIRCRAFT HOVERED IN THE LANDING ZONE TWENTY - THIRTY SEDONDS WHILE THE AIRCRAFT COMMANDER WAS MANEUVERING THE AIRCRAFT, PER INSTRUCTIONS FROM THE CREW CHIEF AND GUNNER, ATTEMPTING TO FIND A SATISFACTORY PLACE TO LAND. THE MAIN ROTOR STRUCK A TREE APPROXIMATELY 18" IN DIAMETER. BOTH BLADES WERE DIAGIONALY TORN AND RIPPEDON THE BLADE ENDS. ONE BLADE HAD 4 INCHES TORN FROM THE LEADING EDGE AND SPLIT 24" IN LENGTH. THE OTHER BLADE HAD 2 INCHES TORN FROM THE LEADING EDGE AND 4 3/4 INCHES TORN FROM THE TRAILING EDGE. THE AIRCRAFT BEGAN TO VIBRATE TO THE EXTENT OF BECOMING UNCONTROLLABLE. CONSEQUENTLY CPT BUTLER REDUCED THE COLLECTIVE PITCH TO THE FULL DOWN POSITION. THE AIRCRAFT LANDED HARD WITH ADDITIONAL DAMAGE. WHEN THE ROTOR BLADES HIT THE TREE THE TRANSMISSION BROKE LOOSE FROM ITS MOUNTS AND THE TAIL BOOM PARTICALLY BROKE OFF. THE LEFT SKID/CROSS TUBES PARTIALLY COLLAPSED AND MAJOR DAMAGE WAS INFLECTED ON THE MAJOR COMPONENTS OF THE AIRCRAFT. FOLLOWING THE LANDING WO KENNY TURNED THE FUEL AND SWITCHED OFF. THE ENTIRE CREW AND PASSENGERS THEN EXITED THE AIRCRAFT.\\
