Accident Summary:
^WO1 KELLY^ ACTING IN HIS CAPACITY AS AC AND ^1LT VAN SLYCK^ ACTING AS PILOT WERE ASSIGNED A DCS MISSION IN SUPPORT OF OPERATIONS AT QUAN LOI, RVN ON 15 NOVEMBER 1971. AT APPROXIMATELY 0530 HRS ON THE MORNING OF 15 NOVEMBER 1971, ^WO1 KELLY^ AND ^1LT VAN SLYCK^ PERFORMED AN ADEQUATE PREFLIGHT OF THEIR AIRCRAFT (66-16467). AT 0700 THEY DEPARTED PLANTATION FOR QUAN LOI AND ARRIVED AT THEIR DESTINATION ABOUT 0800 HRS AT WHICH TIME THEY SHUT THEIR AIRCRAFT DOWN. DURING THE NEXT THREE (3) HRS THEY WERE GIVEN TWO MISSIONS TO FLY BACK AND FORTH TO LOC NINH, BUT ULTIMATELY SHUT DOWN AGAIN AT QUAN LOI AT 1100 HRS. DURING THESE MISSIONS THEY EXPERIENCED NO OPERATIONAL DIFFICULTIES WITH THE AIRCRAFT. THEY ALSO CHECKED ALL OIL LEVELS AND ROTOR HUBS AFTER EACH PARTICULAR PHASE OF FLIGHT. AT APPROXIMATELY 1130 HRS THEY WERE CALLED OUT AGAIN TO PERFORM A MEDEVAC FOR A DOWNED HELICOPTER CREW (VNAF) APPROXIMATELY 5NM FROM LOC NINH. THEY PICKED UP THREE (3) CREW MEMBERS AND DEPARTED THE AREA ENROUTE TO BIEN HOA. WHILE ENROUTE THEY RECEIVED INSTRUCTIONS TO CHANGE THEIR DESTINATION AND DELIVER THEIR PASSENGERS TO TAN SON NHUT AFB FOR MEDICAL ATTENTION. AT 1212 HRS, APPROXIMATELY 5 NM NORTH OF TAN SON NHUT, AT AN ALTITUDE OF 2000' MSL, AN AIRSPEED OF 80 KTS, AND A HEADING OF 180 DEGREES, THE AIRCRAFT EXPERIENCED A HIGH FREQUENCY VIBRATION AND EXTREMELY LOUD GRINDING NOISES COMING FROM THE TRANSMISSION AREA. ^WO1 KELLY^ ASSUMED THE AIRCRAFT HAD A TRANSMISSION FAILURE AND LOWERED THE PITCH. THE AIRCRAFT DESCENDED AT A RATE OF 500 FPM. DURING THIS TIME A MAYDAY CALL WAS MADE BY WO1 KELLY AND ^1LT VAN SLYCK^ LOCKED BOTH SHOULDER HARNESSES AND MONITORED ALL INSTRUMENTS. BOTH AC AND PILOT SAW NO ABNORMAL INDICATIONS ON THE INSTRUMENT PANEL BUT CONTINUED THEIR DESCENT. ^WO1 KELLY^ DETERMINED AT APPROXIMATELY 50 FT AGL THAT HIS APPROACH WAS SHALLOW AND FAST. HE BEGAN TO INITIATE A DECELERATION FROM 80 KTS. AT 10 - 15 FT AGL THE NOSE BEGAN TO YAW TO THE RIGHT AND HE APPLIED LEFT PEDAL, HOWEVER THE AIRCRAFT DID NOT RESPOND. HE HIT A MARSHY AREA HEELS FIRST AT A 45 DEGREE ANGLE TO HIS FLIGHT PATH SKIDDING APPROXIMATELY 10 FEET. DURING THE SKID, THE AIRCRAFT TURNED TO THE RIGHT ANOTHER 45 DEGREES. ALL CREWMEMBERS EXITED THE AIRCRAFT WITHOUT SHUTTING THE AIRCRAFT DOWN. AFTER ALL INDIVIDUALS WERE OUT AND IT WAS DETERMINED THAT THE AIRCRAFT WOULD NOT EXPLODE, 1LT VAN SLYCK RETURNED TO THE AIRCRAFT AND SHUT IT DOWN. COMMUNICATIONS ABOARD THE AIRCRAFT WERE USED TO NOTIFY THE PROPER AUTHORITIES. THERE WAS NO FIRE, THE AIRCRAFT REMAINED UPRIGHT, AND THERE WERE NO INJURIES A QUICK INSPECTION WAS PERFORMED AND IT WAS DETERMINED THAT THE NO. 1 HANGER BEARING HAD FAILED RATHER THAN THE TRANSMISSION.\\
