Accident Summary:

 AT ABOUT 1415 HOURS, 14 SEPTEMBER 1970, 2LT PENNOCK WAS ADMINISTERING A CHECKRIDE TO 2LT TRI. THEY WERE IN TRAFFIC AN HOUR DOING NORMAL APPROACHES AND LANDINGS, AUTOROTATIONS, AND FINALLY SIMULATED ANTI-TORQUE FAILURES. A SIMULATED ANT-TORQUE FAILURE, LEFT PEDAL FULLY APPLIED, WAS DEMONSTRATED AND ONE ATTEMPTED UNSUCCESSFULLY BY THE PILOT. THE IP MADE A RECOVERY AND EXPLAINED THE MANUEVER AGAIN. ON A SECOND TRY, THE PILOT WAS REACTING SATISFACTORILY AND THE AIRCRAFT APPROACHED THE GROUND, AT A 45 DEGREE ANGLE, THE RIGHT SKID STRUCK THE GROUND FIRST. THE AIRCRAFT HAD FORWARD GROUNDRUN, AND IN AN ATTEMPT TO BRING THE AIRCRAFT TO A HOVER THE AIRCRAFT TURNED TO THE LEFT SIDE, WHERE THE MAIN ROTOR BLADE STRUCK THE GROUND. THE AIRCRAFT THEN SETTLED ON THE LEFT SIDE. THE TAIL BOOM WAS SEVERED AND MAJOR DAMAGE WAS SUSTAINED.\\
