Accident Summary:

 THE AIRCRAFT WAS ENROUTE FROM BAN ME THUOT, RVN TO A FIRE SUPPORT BASE LOCATED AT CAM NY, RVN. THE PILOT WAS AT THE CONTROLS AND THE AIRCRAFT'S ALTITUDE WAS APPROXIMATELY 2500 FEET AGL. AT THIS TIME ALOUD NOISE WAS HEARD FROM THE TAIL BOOM AREA AND THE NOSE OF THE AIRCRAFT YAWED RIGHT WHILE THE AIRCRAFT ASSUMED A NOSE-LOW ATTITUDE. THE AIRCRAFT COMMANDER TOOK THE CONTROLS FROM THE PILOT AND REDUCED THE COLLECTIVE TO CORRECT THE YAW AND LOWERED THE NOSE TO STREAMLINE THE AIRCRAFT. AN ATTEMPT WAS MADE TO FLY THE AIRCRAFT TO PHAN RANG AFB LOCATED 10 MILES TO THE EAST BUT THE AIRCRAFT WOULD NOT STREAMLINE SUFFICIENTLY TO ALLOW CONTINUED FLIGHT. DURING THIS PERIOD THE CREW CHIEF HAD CONFIRMED THAT THE 90 GEAR BOX AND TAIL ROTOR BLADES HAD SEPARATED FROM THE VERTICAL STABILIZER. THE AIRCRAFT COMMANDER ATTEMPTED TO PUT THE AIRCRAFT DOWN ON A ROAD BUT A TRUCK CONVOY ON THE ROAD PREVENTED A RUNNING LANDING. THE AIRCRAFT WENT OUT OF CONTROL ON SHORT FINAL AS A FLARE WAS STARTED TO REDUCE THE AIRSPEED. THE AIRCRAFT CRASHED INTO THE TREE LINE ON THE NORTH SIDE OF THE ROAD AND ROLLED OVER ON ITS LEFT SIDE. THERE WAS NO POST-CRASH FIRE.\\
