The insignia on the left is the first crew insignia for REQUIN, as designed by the Walt Disney Studios. The insignia on the right is a reproduction of an original crew patch.

When REQUIN was officially designated as SSR-481 in 1948, the crew insignia was changed to reflect REQUIN's new mission. The patch illustrated below is based on a reproduction of an original version. This insignia was also produced in a large 9-inch jacket-back version as well.

In August of 1959, when REQUIN was redesignated SS-481, a new design for REQUIN's crew insignia was needed. The design on the bottom left was submitted, but came back looking like the version on the bottom right. The right-hand insignia is based on a reproduction of an original version.
