If You Are Self - Employed:
2 years signed personal Federal Income Tax Returns with all Schedules.
1 year - to - date Audited Profit & Loss Statement, if applicable.
2 years signed Federal Corporate Income Tax Returns with all schedules, if
you have a controlling interest.

If Your Income Is Primarily From Commissions:
2 years signed personal Federal Income Tax Returns with all Schedules.

If You Are Retired:
2 years signed personal Federal Income Tax Returns with all Schedules and a
Personal Financial Statement.

If You Are Buying a Condominium or a Planned Unit Development (PURD):
Association Letter.
The Operating Budget of the Association
A Re-Sale Certificate.
A Declaration of Condominium Bylaws.

*A mortgage officer may request additional information in order to process your application.

A CHECKLIST OF ITEMS NEEDED FOR A MORTGAGE APPLICATION

1. Purchase & Sale (fully executed) on the subject property.

2. Purchase & Sale on existing properties, if applicable.

3. MLS Listing; For taxes, book and page.

4. Social Security Numbers

5. Name and address of employers for last 2 years.

6. 2 years of residential addresses.

7. Savings, checking and individual retirement account balances, and bank
name and address.

8. Number and names of shares in stocks and bonds.

9. Life insurance, face amount and cash value.

10. Automobiles owned - make and year.

11. Approximate value of personal property.

12. Loan account numbers, credit cards, auto loans, account numbers, monthly
payments, balances, creditor's names and addresses.

13. Divorce Decree (if applicable). (Alimony may be used as income if
information is volunteered.)

14. Application Fee; to cover credit report and appraisal costs.
