This Cat/Kitten Alteration Contract (herein referred to as Contract) is entered into by

The Purchaser agrees to have the above cat/kitten altered on or before the six month birthdate, or within 30 days if said cat/kitten is over 6 months old, said altering will take place no later then _____/_____/_____ (date). The purchaser will remit to the seller veterinarian verification of altering within one month of the above date, at which time the seller will mail the cat/kitten’s papers within ________ days of notification.

In the event that the Purchaser is found to have violated any of the terms and conditions of this contract, Purchaser shall be liable to Seller’s Cattery for the damages in the amount of the Purchase Price of said cat/kitten plus expenses for veterinary care provided to cat/kitten at that time, plus liquidated damages in the amount of $100, plus costs from litigation of this contract. The cat/kitten is to be surrendered unconditionally to Seller along with any and all offspring of said cat/kitten. Failure on the part of the Purchaser to honor any and all conditions of this contract will result in the ownership of said cat/kitten to revert to Seller.

This one page document is a legally binding contract:

The laws of the state of Arizona shall be controlling.

Any dispute regarding this contract:

Shall be brought before the State of Arizona Court of Law in Maricopa County, the city of Phoenix, Arizona.

No other state or municipality shall have jurisdiction over this contract.

There are no other conditions or guarantees, whether verbal or implied.

Purchaser’s signature indicates full understanding, agreement, and approval of all above terns and conditions.
