To use the KAS Quilts order form the Adobe Acrobat reader is required. If you need to obtain the FREE Acrobat Reader click on the following image to go to the Adobe web site.
When the Acrobat Reader is installed on your computer click on the following image to display the order form.
When the form is completed, you should mail the following items to address below:
- Your quilt
- The Order Form
Payment is only due after
your quilt has been completed.
