TI's TSBKOHCI403. This is the most reliable OHCI card I've seen so far. Good job TI!
NEC µµPD72861 (search for "OHCI" on their web page).
IBM. I do not recommend this card. I tried to install it and my test machine wouldn't even boot (Gateway E-3000, Pentium 200 Mhz). VERY SCARY.
