Your body is a bit like a car - it needs a regular service to keep things running along smoothly. Many medical problems are easy to prevent, and putting off having a thorough health check could mean a problem occuring at an inconvenient time.

Below are a list of questions about your health. Different questions are relevant for different ages, but some like tetanus and skin checks apply to almost all of us. If you find you are answering "no" or "don't know" to most of these questions you may wish to call and make an appointment.

1)  When was your last tetanus booster?

2) Have you ever had a cholesterol test?           When?

3) Have you had a formal skin check for skin cancers?

4) Have you been tested for sugar diabetes?     When?

5) Have you had a thyroid test?     When?

6) Have you ever had high blood pressure?

7) {If over 50} Have you had a bone density test?

8) {If over 50} Have you ever been tested for bowel cancer?

9) {Males over 60} Have you had a prostate check?

10) {Females} When was your last pap smear?

11) {Females} When was your last breast check?

The answers on this survey can be entered into our recall system if you wish and used to generate automatic recalls when tests and immunisations come due. For questions 8 to 11 you may want to specifically see a male or female doctor - please let the receptionist know. You may wish to make a separate long consultation to go through all the above.
