When asked to identify Doctor Robert, Paul replied, "Well, he's like a 
joke.  There's some fellow in New York... we'd hear people say, 'You can get 
everything off him, any pills you want'... [He] cured everyone of everything 
with these pills and tranquilisers... he just kept New York high."

The true identity of Doctor Robert was most likely Dr Robert Freymann, who 
had a practice on New York's East 78th Street.  Many of his clients worked 
in the fields of art and entertainment and one of them was John Lennon, who 
wrote the song.  Freymann was struck off in 1975, having been found guilty 
of malpractice.
