In the pilot, Dr Doug Ross arrived at the E.R. first thing in the morning after drinking all night. He is charming and engaging. He also turns out to be somewhat of a womanizer; a man who is used to his good looks and easy manner taking him where ever he wants to go. Then later Doug surprises us with his tenderness toward his younger patients. He is a compassionate pediatrition who looks into their eyes with empathy and seems to have an understanding of their pain. Whe he turns with fury on the parents who may be abusing their children we can't help but cheer.
In one episode George Clooney's character was described as "a resonably normal guy with sloppy impulse control." This description seems fairly accurate. "Doug Ross is a guy in his mid-thirties," said George," who has just discovered that all the things he used to do in life - partying a little too hard and chasing girls a little too much - are all starting to catch up to him. It's not working like it used to. He is faced with having to deal with being an adult." Doug seems to be dealing with adulthood better now. He is engaged to Carol Hathaway and both of them are very happy.
