With the two-player game, you need to change your strategy. It is a good idea to stay away from the edges of the screen and to share out the power-ups. If you don't then one of the players is going to die pretty soon and you may as well have started a one-player game. Decide before you start who's playing at the front and who's covering the rear then stay close together firing at different monsters.
The code RVDBK0Q&JWM6 lets you start on World 4 with the Preacher and Gentleman both having 30 lives each.
