Top to bottom:
Tenkou: The big baddie from OAV 1 is back and an even bigger baddie. I have no idea what his true intentions are. Nakago once served him. (Oh, that's why. :P)
Yousui: No, that's not Tamahome. Yousui is the fourth pawn of Tenkou. He took the form of Tamahome and his memories as well to fool Miaka and drive Taka out of the picture. While trying to kill Taka, he killed Miaka instead. (she stepped in the way.)
Hikou: Chichiri's best friend from the past. He died several years ago. (read the synopsis for details, plz.) Hikou controls water and decides to use Miaka to test Chichiri's weakness.
Kamishiro Miiru/Miisu A woman who went to Keisuke, Tetsuya, and Taka's college. She planted a parasite in Taka's ear and made a doll out of the blood from it. (Spooky.) She and her brother Ren were put to death for their incestious relationship.
Shigyou Ren: Miisu's little brother (who she was incest with. -.-;;) who went to Miaka's school (and was quite popular, ran for president) and tried to get the students to kill her.
