Shoulder Dystocia:

Shoulder dystocia occurs during childbirth and is due to the babies shoulder lodging.

To deliver a baby that has large shoulders, the doctor may attempt the delivering like shown in the picture. Delivering the head and a shoulder and then proceeding to the other shoulder.
