I was going to insert a workable database here, but decided instead to list the fields and their uses so you could use any database software (that would generate queries) that you chose.
I used Access and found it worked very well. By placing markers (x’s) in the fields listed in the second table, I was able to create queries for all kinds of situations. Field sizes may have to be adjusted in some cases. You can use this as a basis and add your own. All fields can be easily joined into a merge (Word) if you are printing envelopes, etc. They also serve to keep information organized if you are handwriting everything.
The main table can also be used for Christmas cards simply by adding a field for it and placing a marker in that field in order to generate the proper query.
