I certify, on honor, That Joseph Chapman, a corporal of Captain Brakens' Company {D} of Indiana, born in Mason Co., State of Kentucky, aged 53 years; 5 feet 10 inches high; Dark complexion, dark eyes and hair, and by occupation a farmer, was mustered into service of the United States at Madison, Indiana on the 8th day of October 1847, to serve --------- months or for during the war with Mexico.

Certificate of Disability and Discharge, in case of a Volunteer soldier who may be discharged before expiration of his term, in consequence of wounds recieved or sickness incurred in cours of service
I hereby certify, that Joseph Chapman, a corporal of company, { D } of the Fifth Regiment of Indiana Volunteers, is incapable of performing the duties of a Volunteer soldier, for the following reasons: to wit; While actually engaged in service during the month of December last was attacked with remittent fever which has resulted in general prostration of the system.
And I further certify, That in the opinion of the undersigned, the public interest requires that the said Joseph Chapman be discharged from the Volunteer service of the United States.
Given at the City of Mexico this 7th day of February 1848
