In case of accident


If you have a pet in the car with you and you get into an accident, you may be unable to take care of your pet. To make sure your pet is cared for as you like, prepare an emergency instruction note for your pet and place it in your wallet with your driver’s license so that emergency personnel will find it.

In the event that I, [YOUR NAME], am incapacitated and unable to make my wishes known regarding my [CAT/DOG/OTHER], [PETNAME], please honour the following requests:

“[EMERGENCY CONTACT PERSON] is to be contacted as soon as possible at [PHONE]. If they cannot be reached, contact [ALTERNATE CONTACT] at [PHONE]. All expenses for [PET NAME] will be guaranteed by them. If [PET NAME] is not injured and the above people cannot be reached, they are to be cared for by my veterinarian listed below or the nearest reputable boarding kennel as the last resort, and be kept in the best possible manner until arrangements can be made to get them home. I prefer that my veterinarian [VET’S NAME AND PHONE NUMBER] be contacted regarding decisions on [PET’S NAME] care and treatment. If my pet is injured beyond all hope of recovery, he or she is to be humanely euthanized. A photograph of [PET’S NAME] is attached. [PET’S NAME] is a [AGE]-year old [MALE/FEMALE], [BREED], [COLORS]. [LIST ANY TATTOOS OR IDENTIFYING MARKS].
