Name/Gender Change on Driver's License

POLICY
An applicant may have the sex designation shown on the
current driver license, instruction permit, or identification card changed
if they have undergone sex change surgery or they are a transsexual undergoing
gender reassignment therapy with a qualified therapist.
The applicant must present documentation from any physician
stating the applicant underwent sex change surgery or a letter
from a qualified therapist staling the applicant is living full-time
as the desired gender as pan of gender reassignment therapy.
(Exception: No verification is necessary if the change is to correct
a typographical error on the current driver license, instruction
permit or identification card. Do not charge a duplicate fee to
correct a typographical error.)
If, after changing the sex designation, the applicant decides
that he/she no longer wishes to continue the gender reassignment therapy,
the applicant must present a letter from any physician indicating that the
applicant is no longer participating in gender reassignment therapy.
If an applicant with a previous Oregon record moves
back to Oregon and underwent sex change surgery outside of Oregon, the applicant
must present a letter from any physician verifying the surgery.
If an applicant with a previous Oregon record moves
back to Oregon and began the gender reassignment therapy outside of Oregon,
the applicant must present a letter from a qualified therapist slating the
applicant is living full-time as the desired gender as part off gender
reassignment therapy.
Charge the appropriate duplicate fee. If the
applicant is eligible to renew, charge only the renewal fee.
If the customer is also changing his/her name,
the applicant must present one proof of identification in
the previous name and one proof of identification in the new name. You
may accept the letter from the therapist as proof of the new name if the
new name information is included in the letter.
Use the customer identification number previously
assigned to the applicant.
PROCEDURE
View documentation from the physician stating the
applicant underwent sex change surgery or stating the applicant is living
full-time in the desired gender.
If the applicant is also changing his/her name, verify
applicant's proof of identity. The applicant must submit one proof in the previous
name and one proof in the new name.
Return the documentation to applicant.
Correct the sex designation in the "Sex" field in NLS.
Process as a duplicate or renewal, as appropriate.
