Yaakov comes from the Hebrew word 'akev' meaning 'heel'. The other meaning is 'supplanter' which probably comes from the story about Yaakov (later known as Jacob and also Israel). The biblical Jacob was born holding on to his brother Esau's heel. He was the son of Isaac and Rebecca and the father of the twelve founders of the twelve tribes of Israel.
