i've seen several of these things on the net, and when i saw one on my buddy Rich's Myspace, i decided that i needed to make my own. this is the result of that decision, combined with a few minutes of research on google and 30 seconds of soul–searching about my own priorities.
the method of completing the application is up to you. it's probably easiest to copy/paste the questions into a Word document, fill it out at your leisure, and then copy/paste it into an email. you should consult the disclaimer and guidelines and then submit the completed application and an appropriate picture by email.
